Parental Consent: For Girls under 13 Years Old to Participate or Communicate in DoubleSunrise.com's Surveys, Forum and Feedback

Parental/Guardian Consent Form Your daughter has requested permission to participate in GirlTalk (or to communicate to DoubleSunrise.com), our forum for girls to comment, offer feedback, and complete surveys regarding girls' health. The purpose of offering, for example GirlTalk, is in line with our motto, "Keeping Connected" in that it allows her to communicate to us and develops a two-way conversation. Our aim is to choose common situations and issues for girls to express their views. A secondary gain is that we will read the responses internally and the responses may give us informal validation (or invalidation) of what we hear clinically from girls.

In regulation with COPPA (Children's Online Privacy and Protection Act), all children under the age of 13 must have parental consent before offering any personal information over the Internet. Because participation in GirlTalk requires an e-mail address, it is considered personal information and parental consent is necessary. An e-mail address is the only required personal information to participate in GirlTalk. Any other information offered (i.e., name, address, age) is not required nor is it recommended that such information be provided, but is solely at the discretion of your daughter. Please be aware that DoubleSunrise will only use personal information for internal purposes. Any information we gather will not be sold or distributed to third parties.

It is our intent to provide a safe and helpful haven for your child to gather information, share stories, and answer surveys. We intentionally request as little information as possible in an attempt to protect her privacy. If you have any questions regarding this form, you may refer to the Notice for Parents.

Welcome to DoubleSunrise. Our site is as much for you, the parent, as it is for your daughter(s). Our goal is to provide accurate, up-to-date, and relevant health care information is a manner that is safe and respectful. Some of the topics covered include nutrition, relationships, bodily changes at puberty, warning signs for those at risk, and many, many more.

Naturally, your daughter's protection is a high priority. We are also committed to her protection and adhere to the Federal Trade Commission's Children's Online Privacy and Protection Act (COPPA). Under this act, all children under the age of 13 are required to obtain parental or guardian consent before issuing any private information over the internet that could possibly be obtained by those with less desirable motives. Your daughter is free to browse our site without any personal information required. However, should she wish to participate in GirlTalk or communicate with DoubleSunrise.com in the form of submission of any stories, comments or survey answers, this would result in personal information being transmitted over the Internet. The only information required is an e-mail address; other personal information (i.e., name, address, age) is not requested and can only be obtained if your daughter chooses to initiate it. As always, we encourage you to discuss online safety issues with her and keep personal information private.
